Online Incident Reporting


Context

The paper based F18 Incident Report Form has been in use for many years and not changed in format since May 2015. 

Paper forms carry an inherent risk of loss, give rise to a time delay in completion and submission, and are not a contemporary method of reporting, tracking and responding to incidents and near misses. This has been raised as a significant risk factor by insurers.

Scouts Queensland has now introduced an online Incident Report Form.

This online incident report form is not for reporting child protection issues. That remains covered by completion of the F8 Child Abuse Report Form.

Purpose

The purpose of this document is to 

  1. provide you with guidance on location of the online form, 
  2. completion and submission of a cleaner form, 
  3. transition arrangements from the paper F18 and to clarify when it should be completed. 

Where is it?

The online Incident Report Form is publicly accessible from the menu ribbon on the scoutsqld.com.au home page.

It is deliberately not locked behind any member login requirements to ensure it is easily accessible. The form can now be completed and submitted from a smart phone, tablet or computer. 

What has changed?

The Incident Report Form has been modified to remove collection of redundant information at the initial reporting stage making it simpler and quicker to complete. Should additional information or further follow up be required that will be sought following submission. 


When should I report an incident?

We encourage reporting of any incident or near missesIf in doubt submit an incident report.

Some incidents require immediate notification. The form contains prompts for incidents which require immediate notification, by phone, to the emergency line on 07 3870 7000 (select option 9).

Making the form available online improves the ability for it to be submitted soon after the initial response to the incident. 


Can I still use paper forms?

Paper F18s will continue to be available and accepted as we all transition to the online form. The paper F18 available at docs.scoutsqld.com.au will include a link to the online form to encourage its use. To assist with the transition, each time a paper form is submitted the acknowledgement of receipt will include a reminder of the need to use the online form going forward.
