Training - Scouts Queensland Child Safety Incident Report Form

The below links are replicas of the Scouts Queensland Incident Report Form and the Scouts Queensland Child Safety Incident Report Form.

These training forms will be substantially the same as the online reporting forms but may differ in small ways. These versions of the forms are intended to be used for the following purposes (as applicable):

  1. On training course to help explain how to complete the form
  2. At any time to improve understanding of how to complete the form
  3. To create sample Incident Reports for use in portfolios when seeking VET qualifications
  4. To create mock Incident Reports to support completion of OAS I-Statements (eg Stage 5 Aquatics – Lifesaving)

To print either form:

  1. Click ‘Save my response to edit’ at the bottom of the last page
  2. Click on the form in the Recent tab (older forms will be in the Filled Forms tab)
  3. In the form, right click and select Print.


Real incidents must be reported as required using the Online Incident Reporting on the Scouts Queensland website.
